近日,新華網邀請北京市西城區平安醫院院長、精神科主任醫師肖存利做客新華會客廳,從醫學和心理學的角度為網友們剖析失眠背後的原因,分享改善的建議和方法。

新華網:能否請您簡單解釋一下失眠的定義和症狀?失眠在人群中的普遍程度如何,哪些人群更容易受到失眠的困擾?
肖存利:在臨床上,我們通常定義失眠為:在zai無wu明ming顯xian誘you因yin的de情qing況kuang下xia,一yi個ge人ren若ruo連lian續xu一yi個ge月yue,每mei周zhou至zhi少shao有you三san次ci出chu現xian難nan以yi入ru睡shui或huo維wei持chi睡shui眠mian的de情qing況kuang,則ze視shi為wei患huan有you失shi眠mian症zheng狀zhuang。在zai症zheng狀zhuang上shang,患huan者zhe的de失shi眠mian表biao現xian有you很hen多duo種zhong,比bi如ru有you入ru睡shui困kun難nan,有you睡shui眠mian比bi較jiao淺qian,還hai有you早zao醒xing。同tong時shi,失shi眠mian也ye對dui患huan者zhe造zao成cheng很hen大da的de困kun擾rao。有you的de時shi候hou讓rang他ta自zi己ji很hen難nan受shou,有you的de時shi候hou影ying響xiang到dao患huan者zhe的de人ren際ji關guan係xi、工作能力、周圍環境的識別、注意力等等。

新華網:導致失眠的主要原因有哪些?是否有一些常見的生活習慣或環境因素可能引發失眠?
肖存利:失(shi)眠(mian)的(de)因(yin)素(su)有(you)很(hen)多(duo)種(zhong),與(yu)生(sheng)物(wu)學(xue)有(you)關(guan),也(ye)跟(gen)環(huan)境(jing)有(you)關(guan)。人(ren)類(lei)天(tian)生(sheng)的(de)生(sheng)物(wu)基(ji)因(yin)就(jiu)決(jue)定(ding)了(le)睡(shui)眠(mian)的(de)基(ji)本(ben)規(gui)律(lv)。比(bi)如(ru),兒(er)童(tong)和(he)老(lao)年(nian)人(ren)相(xiang)較(jiao)於(yu)其(qi)他(ta)年(nian)齡(ling)段(duan)的(de)人(ren)往(wang)往(wang)睡(shui)眠(mian)時(shi)間(jian)更(geng)長(chang),通(tong)常(chang)兒(er)童(tong)睡(shui)眠(mian)更(geng)深(shen)沉(chen),而(er)老(lao)年(nian)人(ren)則(ze)可(ke)能(neng)睡(shui)眠(mian)較(jiao)淺(qian)。然(ran)而(er),當(dang)生(sheng)物(wu)節(jie)律(lv)受(shou)到(dao)外(wai)部(bu)環(huan)境(jing)的(de)幹(gan)擾(rao)時(shi),便(bian)容(rong)易(yi)引(yin)發(fa)失(shi)眠(mian)。以(yi)從(cong)一(yi)個(ge)時(shi)區(qu)(如中國)旅行至另一個時區(如美國)weili,zhezhongkuayueshiqudelvxinghuidaozhishengwujielvdezhengtibianhua。ruguoxindehuanjingzhongguangzhaoshijianguochang,huozheheiyeshijianguochang,douhuipohuaiyuanbendeshengwujielv,daozhishimian。chixudebaizhouhuoheiyezhuangtaidouhuishirendeshengwujielvfashengbianhua,jineryinfashuimianmoshidegaibian。

明確失眠原因 進行針對性治療
新華網:目前,對於失眠有哪些常見的診斷方法和評估標準?
肖存利:從(cong)症(zheng)狀(zhuang)層(ceng)麵(mian)來(lai)看(kan),當(dang)一(yi)周(zhou)內(nei)出(chu)現(xian)三(san)次(ci)以(yi)上(shang)睡(shui)眠(mian)問(wen)題(ti),並(bing)在(zai)次(ci)日(ri)產(chan)生(sheng)明(ming)顯(xian)影(ying)響(xiang)時(shi),應(ying)關(guan)注(zhu)睡(shui)眠(mian)質(zhi)量(liang)。在(zai)評(ping)估(gu)睡(shui)眠(mian)時(shi),常(chang)用(yong)的(de)方(fang)法(fa)包(bao)括(kuo)睡(shui)眠(mian)監(jian)測(ce)和(he)量(liang)表(biao)評(ping)估(gu)。睡(shui)眠(mian)監(jian)測(ce)通(tong)常(chang)在(zai)醫(yi)療(liao)環(huan)境(jing)中(zhong)進(jin)行(xing),而(er)量(liang)表(biao)評(ping)估(gu)公(gong)眾(zhong)可(ke)以(yi)自(zi)行(xing)測(ce)試(shi),例(li)如(ru)在(zai)線(xian)搜(sou)索(suo)相(xiang)關(guan)自(zi)測(ce)量(liang)表(biao)。

一(yi)般(ban)來(lai)說(shuo),如(ru)果(guo)躺(tang)在(zai)床(chuang)上(shang)超(chao)過(guo)一(yi)個(ge)小(xiao)時(shi)仍(reng)未(wei)能(neng)入(ru)睡(shui),即(ji)是(shi)入(ru)睡(shui)困(kun)難(nan)。不(bu)同(tong)人(ren)的(de)入(ru)睡(shui)速(su)度(du)差(cha)異(yi)很(hen)大(da),有(you)的(de)人(ren)可(ke)能(neng)很(hen)快(kuai)入(ru)睡(shui),而(er)有(you)的(de)人(ren)則(ze)可(ke)能(neng)長(chang)時(shi)間(jian)無(wu)法(fa)入(ru)睡(shui)或(huo)突(tu)然(ran)驚(jing)醒(xing),這(zhe)些(xie)症(zheng)狀(zhuang)在(zai)臨(lin)床(chuang)上(shang)都(dou)很(hen)常(chang)見(jian)。另(ling)外(wai),一(yi)些(xie)人(ren)在(zai)淩(ling)晨(chen)3、4點鍾就醒來,這可能與多種疾病有關,包括有精神科常見的抑鬱、焦慮、輕度睡眠障礙,嚴重的甚至可能有精神疾病,如精神分裂症等。
還有一些失眠是由內科疾病引起的,比如器質性疾病、呼吸暫停綜合症、心髒科的一些疾病、疼痛等。在診斷失眠時,首先需要排除這些潛在的內科疾病因素。在確認沒有這些疾病影響的情況下,才將失眠視為一個獨立的、單純的睡眠問題。

新華網:在治療失眠時,您通常會推薦哪些方法?對於不同原因導致的失眠,治療方法是否會有所不同?

正確認知失眠 避免誤區
新華網:老百姓在生活當中會遇到各種各樣的失眠問題,可能有一些誤區所在,您在臨床當中有沒有遇到失眠誤區呢?
肖存利:關於失眠和藥物成癮,許多人存在一些誤解。首先,藥物成癮通常發生在長期、大da量liang使shi用yong某mou種zhong藥yao物wu的de情qing況kuang下xia,例li如ru一yi天tian服fu用yong超chao過guo推tui薦jian劑ji量liang的de百bai安an多duo類lei藥yao物wu,或huo者zhe長chang時shi間jian使shi用yong短duan效xiao藥yao物wu如ru三san唑zuo侖lun和he咪mi達da唑zuo侖lun。為wei避bi免mian成cheng癮yin風feng險xian,這zhe類lei藥yao物wu應ying在zai專zhuan業ye醫yi生sheng的de指zhi導dao下xia使shi用yong,且qie使shi用yong時shi間jian不bu宜yi過guo長chang。現xian在zai有you許xu多duo中zhong效xiao或huo長chang效xiao藥yao物wu可ke供gong選xuan擇ze,對dui於yu嚴yan重zhong焦jiao慮lv的de患huan者zhe,醫yi生sheng會hui推tui薦jian使shi用yong長chang效xiao藥yao物wu。
